Wellness Reimbursement Program
Employees who are eligible for medical, dental and vision coverage are eligible for reimbursement of wellness activities the first of the month following 60 days of employment with Laradon. Laradon will reimburse for wellness activities up to $200.00 per fiscal year per employee. An amount for reimbursement is established at the beginning of the fiscal year to be used by the entire agency. Employees will be required to complete the Wellness Reimbursement Form for reimbursement. Activities that Laradon will reimburse for are as follows:
• Health Club Memberships
An employee requesting reimbursement for Health Club Memberships will have to provide supporting documentation that they participated in wellness activities at the Health Club no less than 10 times in a month (the first of the month to the last day of the month).
Laradon will reimburse up to $40.00 a month per employee. The employee will be required to present a receipt for the total annual cost of the membership. This annual cost will be broken down into a monthly reimbursement rate and the employee will be reimbursed that amount up to $40.00 a month and a total of $200.00 per fiscal year.
• Organized Fitness Activities (i.e. walks, runs, triathlons, cycling, etc.)
An employee requesting reimbursement for an organized wellness activity will have to provide a receipt for the event (the first of the month to the last day of the month). Laradon will reimburse up to $40.00 a month per employee up to $200.00 per fiscal year.
The reimbursement will be added to the employee’s pay check and the employee will be taxed on the benefit.
