GENERAL PROGRAM ADMISSION CRITERIA

1. Referrals to Laradon must be within the following age requirements:

2. The person being referred must be diagnosed with developmental disability and/or other disabilities, such as traumatic brain injury or medical conditions. Children 6 weeks through 3 years of age may be diagnosed as at risk for developmental delays or with a medical condition.

3. No person shall be admitted with a disease in a communicable form without an evaluation by a multidisciplinary team, the Admission and Discharge Committee and approval from the Deputy Director.

4. Challenging behaviors must be controllable by structured environment, behavior programming and medication for diagnosed psychiatric disorders.

5. The person being referred must have adequate funding, either public or private.

Individual Employment Program

Entrance Criteria

  1. Meets Laradon's General Program Admission Criteria.
  2. Is eligible for Community Integrated Employment (CIE) funding and/or Division of Vocational Rehabilitation (DVR) funding.
  3. Has expressed and demonstrated a desire to obtain community placement.
  4. Has an Service Plan (SP) or an Individual Employment Plan directed toward community employment.
  5. Has attitude of positive regard toward fellow workers and acceptance of supervisory authority.
  6. Requires minimal supervision after initial instruction on new job.
  7. Meets employer's attendance and punctuality requirements.
  8. Is able to communicate wants and work needs and seeks supervisory assistance when needed.
  9. Is able to stay at work station without supervisory interaction.
  10. Is able to attend to task.
  11. Has the emotional and physical stamina to work required hours.
  12. Practices good hygiene skills.

Exit Criteria

  1. Primary referral agency declares participant ineligible for further services.
  2. The participant requests transfer to another program within Laradon.
  3. The participant loses community placement and is referred to another program within Laradon for further training.
  4. The participant voluntarily discontinues services.
  5. The participant's funding changes.
