GENERAL PROGRAM ADMISSION CRITERIA

The referral must be diagnosed with developmental disability and/or other disabilities, such as traumatic brain injury or medical conditions. Children 6 weeks through 3 years of age may be diagnosed as at risk for developmental delays or with a medical condition.

No person shall be admitted with a disease in a communicable form without an evaluation by a multidisciplinary team, the Admission and Discharge Committee and approval from the Deputy Director.

Person should not be injurious to themselves or others.

Problem behaviors must be controllable by structured environment, behavior programming and medication.

The referral must have adequate funding, either public or private.

Individual Employment Program

Entrance Criteria

  1. The age requirement is 18 years and older.
  2. Is eligible for Community Integrated Employment (CIE) funding.
  3. Meets the Laradon Program Admission Criteria.
  4. Has expressed and demonstrated a desire to obtain community placement.
  5. Has an Individualized Plan (IP) goal directed towards community placement.
  6. Has attitude of positive regard toward fellow workers and acceptance of supervisory authority.
  7. Requires minimal supervision after initial instruction on new job.
  8. Is punctual, preferably early to work and on time returning from lunches and breaks. Also, reliable in the work environment.
  9. Is able to communicate wants and work needs and seeks supervisory assistance when needed.
  10. Is able to stay at work station without supervisory interaction.
  11. Is able to attend to task.
  12. Has the emotional and physical stamina to work required hours.
  13. Has good attendance, doesn't miss more than two days per month.
  14. Knows and practices good hygiene skills.

Exit Criteria

  1. Primary referral agency declares participant ineligible for further services.
  2. The participant requests transfer to another program within Laradon.
  3. The participant loses community placement and is referred to another program within Laradon for further training.
