GENERAL PROGRAM ADMISSION CRITERIA

The referral must be diagnosed with developmental disability and/or other disabilities, such as traumatic brain injury or medical conditions. Children 6 weeks through 3 years of age may be diagnosed as at risk for developmental delays or with a medical condition.

No person shall be admitted with a disease in a communicable form without an evaluation by a multidisciplinary team, the Admission and Discharge Committee and approval from the Deputy Director.

Person should not be injurious to themselves or others.

Problem behaviors must be controllable by structured environment, behavior programming and medication.

The referral must have adequate funding, either public or private.

Community Participation Program

Entrance Criteria

  1. The age requirement is 18 years and older.
  2. Meets the Laradon Program Admission Criteria.
  3. Has an IP goal that is appropriate to curricula.
  4. Can physically and emotionally tolerate a minimum of a three-hour workday.
  5. Can follow a simple a.m. routine (e.g., hang up coat, punch in on the clock, sit in seat).
  6. Can work continuously on tasks for at least five minutes (normal pauses accepted).
  7. Can perform a two-step task (e.g., pick up the block and put it in the box).
  8. Can follow a two-step verbal or gestural mobility command (e.g., "go to lunch," "go the bus," etc.).
  9. Exhibits some knowledge and awareness of basic community safety skills.
  10. Can adjust to change in assignment.
  11. Knows and practices good hygiene skills

Exit Criteria

  1. The participant or IDT team requests transfer to another program within Laradon.
  2. The participant is referred to another agency and accepts.
  3. Participant voluntarily discontinues services.
  4. Primary referral agency declares participant ineligible for further services.
