GENERAL PROGRAM ADMISSION CRITERIA

1. Referrals to Laradon must be within the following age requirements:

2. The person being referred must be diagnosed with developmental disability and/or other disabilities, such as traumatic brain injury or medical conditions. Children 6 weeks through 3 years of age may be diagnosed as at risk for developmental delays or with a medical condition.

3. No person shall be admitted with a disease in a communicable form without an evaluation by a multidisciplinary team, the Admission and Discharge Committee and approval from the Deputy Director.

4. Challenging behaviors must be controllable by structured environment, behavior programming and medication for diagnosed psychiatric disorders.

5. The person being referred must have adequate funding, either public or private.

 

Community Participation Program

Entrance Criteria

  1. Meets Laradon's General Program Admission Criteria.
  2. Has an SP goal that is appropriate to curricula.
  3. Can physically and emotionally tolerate a minimum of a three-hour activity.
  4. Can follow a simple a.m. routine (e.g., hang up coat, sit in seat).
  5. Can work continuously on tasks for at least five minutes (normal pauses accepted).
  6. Can follow a two-step verbal or gestural command.
  7. Exhibits some knowledge and awareness of basic community safety skills.
  8. Can adjust to change in assignment.
  9. Knows and practices good hygiene skills.

Exit Criteria

  1. The participant or IDT team requests transfer to another program within Laradon.
  2. The participant is referred to another agency and accepts.
  3. Participant voluntarily discontinues services.
  4. Primary referral agency declares participant ineligible for further services.
  5. The participant's funding changes.
  6. Unexcused absence of more than 15 of schedule program over one month period.
